For Pacific Source and other insurance providers, I bill out-of-network and can provide a superbill for you to submit to your insurance for potential reimbursement.

For medical massage treatment for a motor vehicle accident:

• You must have an active claim number through your auto insurance.

If you’re paying out of pocket, please choose the appropriate self-pay option instead.
